Most people before the 1950's just paid out of pocket. Doctors weren't all entitled to get filthy rich back then, just a few highly skilled surgeons and researchers got wealthy off of medical care. Doctors' visits weren't expensive, and neither were most meds. The military families were the only ones I knew of that had health insurance; very few companies offered it.
