I looked at the form. Since there is no verification means on it, you could mark however you like then provide a false name and signature just to placate the Minions of Ignorance Harris has making you fill it out. It's not like they're going to check. It's sort of like being given a mail-in ballot to fill out and hand back to a DNC worker...
