Why would they? All the state controls is how much they reimburse the provider. The provider can still charge above that reimbursement rate if they think their skills are worth it and that patients will pay extra for them. Those types of doctors exist in almost every single payer system around the world. I'm sure there are plenty of people who will spend the extra money to get the health care they think they need. Personally, I think those people are morons, but I'm not gonna stop them from punching themselves in the nuts.

The state will establish a reimbursement rate that doctors can either accept, or charge above that rate if they think their skills are worth it. Medicare already does this.

Here's the thing...

Right now, according to the Kaiser Family Foundation, it costs about $22K to insure one worker for a full year at any given company. For a company of 50 workers, that is $1M+ spent on health care. The worker pays about $6K of that, and the employer pays the remaining $16K, so a company with 50 employees costs $800K to insure every worker. Remember that cost because it's going to be important later in this post.

The Sanders M4A bill would do away with premiums and copays entirely, and substitute those with a flat payroll tax of 4% on income above $29,000. The average HH income is $70K, so the average worker would pay 4% of $40K ($70K-$30K) or about $1,600. Under the current system, the worker pays $6K. Last time I checked, $1,600 < $6,000.

For employers, they would pay 7.5% on income -not revenue, income- above $100K instead of shelling out $16K for every worker. For a company of 50 workers and for most small businesses, the average income is $53K...so the employer would not pay any tax in order to provide their workers with health care coverage.

This system will result in everyone being covered, there would be no gaps in coverage, and it would include dental and vision.
