They didn't in the past. Fortunately that is changing but it is still being done.

Leave the intersex babies and children alone.

Same with transgender children.
 
If I offer a two year old a $500 bill or a big stack of oreos, which one are they going to choose? That age is not capable of "choosing" their gender...:)

A gay friend of mine said he knew he felt "different" at a very early age. I assume gender dysphoria is similar and perhaps even more foundational to the person's sense of self.
