What they're describing is an amendment to the Social Security act. Here's where they say this language is an amendment to that bill:
1) IN GENERAL- Section 1861 of the Social Security Act (42 U.S.C. 1395x) is amended--
(A) in subsection (s)(2)--
(i) by striking ‘and’ at the end of subparagraph (DD);
(ii) by adding ‘and’ at the end of subparagraph (EE); and
(iii) by adding at the end the following new subparagraph: [followed by a very long amendment detailing what advance care or end-of-life counseling is - the link to which I provided just above]
If these changes make stuff in that section mandatory, you can expect every senior to be forced to have artificial legs and midwife services.
This section of the Social Security Act deals with reimbursement from Medicare only. It just means that if doctors council people on these things, Medicare will pay them to do it as long as it's not more frequently than every 5 years unless certain conditions which they describe are met (like a change in health conditions that would warrant such a consultation - like getting cancer).
