I suggest a real quote box altering rule.

I agree to the extent that open threats of violence against a board member, or for that matter, anyone else should be grounds for a ban, be it temporary or permanent.

As for quoting someone, I see this as you MUST put up the original quote in its entirety, unless you are addressing one specific part of it. Quoting out of context should be disallowed. One should be able to highlight or otherwise emphasize part of a quote if that helps make it clear what you are responding to.

Changing quotes, making up quotes, etc., in someone's board name, using and altering their avatar, or otherwise attempting to make someone say something they never did should be grounds for a ban, along with doing such for the purposes of personal attack on the individual. It's one thing to troll some other member with sarcasm, snark, or flippant remarks. It should be unacceptable to allow anyone to make threats or statements that call for harm to another poster on the board.
