Guns are also designed to hit the target that they are aimed at. The real question is do you think Baldwin intended to discharge a live round at her. No I do not. But I do think he certainly aimed at her and pulled the trigger. He likely expected to discharge a blank.

It is possible it was a single action pistol. That means the hammer might have been half cocked or fully cocked before it discharged. A accidental discharge is more likely if the gun was already cocked. We also don't know if the pistol had a safety on it.

If it was an early western its highly likely it was a revolver and had no safety.

The safety on early single-action pistols, muskets, and rifles is the half-cock. When the weapon is half cocked, it cannot be fired. The hammer has to be fully cocked before it can fire. If the hammer is in the down position, it must be manually cocked to fire. A trigger pull won't do it. Only double action pistols and rifles will fire from the down position on a trigger pull.
